Bi-Lingual Parent Educator/Case Manager CHIP of South Hampton Roads

Make a difference in the life of a child! Home Visiting Parent Educator needed for a dynamic non-profit service provider in South Eastern Virginia. Position provides evidenced based, case management for low income, high need pregnant women and teens, and families with children 0-6. Work is mainly done in the homes of the families we serve. Parent Educators work as a member of a two-person team with an RN. Must be able to provide resources and support, while providing accurate child development, positive discipline and healthy living skills to families with little support. 

Must be a self-starter, organized and able to work on a team effectively.

BA/BS in related field required. 1-year experience in field, documentation skills, computer literate in MS suite, experience in home visitation preferred. Parents as Teachers (PAT) certification preferred, but not required. Spanish skills a plus!

Must be able to provide resources and support, be self-starter, well organized, while providing accurate child development, positive discipline and healthy living skills to families with little support.

Community Health Nurse-RN, CHIP of South Hampton Roads

Description: 

Make a difference in the life of a child! F/T Home Visiting RN will provide ongoing health supervision for low-income pregnant women and families with children 0-6 to ensure healthy outcomes. Position reports to the CHIP Team Supervisor and collaborates with Parent Educators to provide comprehensive service delivery to families on caseload.

Essential Functions:

• Initiate and maintain regular and long-term contact with assigned families.

• Provide direct services to prenatal and postnatal women and families in the family’s home to ensure proper use of medical home, provide health education and support.

• Provide family-centered, strength based interventions as required.

• Maintain case management documentation and required updated reports on families served.

• Act as a liaison between the medical home and the family. Accompany and/or provide transportation for families to medical appointments when necessary.

Education/Experience:

• RN or BSN required w/current Virginia State License as a Registered Nurse or Licensed Nurse

Additional Requirements:

• Knowledge of and expertise in providing home-based services.

• In-depth knowledge of fetal and child development, human sexuality.

• Strong interpersonal skills and cultural sensitivity.

• Current driver’s license and willingness to use own mode of transportation.

• Ability to work well within a team.

• Ability to establish and maintain programmatic boundaries while being flexible and supportive of families.

• Bilingual in English/Spanish a plus.

Possess and maintain good physical stamina and mental health: Usual Days and Hours 

M-F 8:30-4:30
